Hi Steve! I have 2 daughters (21 & 15) with non-classic CAH. The oldest is a petite girl, while the youngest is short in stature but overweight. Our doctor finally admitted that high doses of hydrocortisone will cause an increase in appetite, and cause weight-loss to be virtually impossible. He did some testing with her and said that all her levels were in the "normal" range except the free testosterone which was about 3x higher than it should be. So he switched her from 45 mg cortef to 15 mg prednisone and did what he termed "stacking" of meds. He told her to take one tab morning & 2 tabs at bedtime. We went back after about 6 weeks and this method brought her free testosterone done to "mid" normal levels. So he decided if stacking the meds at night made that much difference, he'd try reducing her to 1 tab at bedtime. We go back in June for more blood work to see how that is working. That way he can reduce her hydrocortisone, and possibly begin working on the losing of weight. Hope this might give you some insight to what we have been thru, and how we are managing. Both my girls were diagnosed at age 3, so you might say we've been at this a while. I must admit, they have come a long way in the treatment of CAH in the last 17-18 years and even more so in just the last few years. Good luck to you.
