Hi Nicole...I found it! This is the letter I posted in March. Good Luck, Diane These are the steps I took with the enormous help from my sister. She unfortunately had to do this for her son, spending a lot of time on the phone, learning laws and loop holes. I have learned a lot from her. This is only a small fragment of what she has taught me. I wish everyone had a sister like her....she is my rock! First we live in Pennsylvania, so check because your state may be different. 1) First you must call your counties welfare office tell them you want to qualify for SSI (Supplemental Security Income) you can either go to the office or set up a phone interview, do the phone interview and make sure you will be home to take the call You will need: name, address, phone #, your household income , your child’s social security number (and yours) When they tell you do not meet because of income, tell them you want a WRITTEN DENIAL they will ask if you need medical assistance for your child, say yes they will tell you that they will send you the application, ask them to ALSO send you the TEDDY BEAR FORM (this is the shorter version, trust me when you see it you will be grateful) 2) When you get the Teddy Bear form fill everything in EXCEPT ANY AND ALL QUESTIONS THAT ASK YOU TO PUT A DOLLAR FIGURE TO IT, example: list any unpaid medical bills, paid medical bills, income, child care expenses, other income such as support, ect. IN THESE SECTIONS WRITE THE FOLLOWING: NOT APPLICABLE MEDICAL ASSISTANCE FOR DISABLED CHILD HANDBOOK SECTION 355.4 MAKE COPIES OF EVERYTHING...WRITE DOWN WHO YOU TALK TO AND WHAT DAY AND TIME IT WAS, WRITE DOWN IF THEY ARE NOT IN AND HAVE TO RETURN YOUR CALL......KEEP A LOG OF EVERYTHING 3) NOW YOU HAVE TO GO TO YOUR COUNTIES WELFARE OFFICE THIS IS WHAT YOU WILL NEED your drivers license you, your spouses social security cards your child’s birth certificate your child’s social security card the denial letter from welfare the teddy bear form completed a letter from your pediatrician AND from the endocrinologist about your child’s medical condition any information you can get about CAH, example I brought with me the MAGIC Foundation’s pamphlet and the one from the NADF (National Adrenal Diseases Foundation, Inc) remember that this is not like Diabetes people DO NOT understand CAH you MUST educate your case worker properly so they will help you and don’t forget your child, they want to see them in person so they know this is all on the up and up!!!! They will then process everything and you will get a letter of acceptance which is great!!! Or DENIAL...my son was accepted so I don’t know exactly what to do if denied out right (but I could find out) I know you can appeal After being accepted my son’s needed to go against a review board. They will send you more paperwork. You need to be very careful on how you word things and to give good, solid information. Again send copies of pamphlets and make copies of the papers you fill out. You can also contact the insurance commissioner of your state. There are also attorneys that are disability advocates. I called and she called back within 1/2 hour. She told me word for word how to answer each question. She also told me that id he was denied and had to go to Harrisburg in front of the review board that they would supply the legal defense at NO COST to me. If my son needed this than they would to it for FREE! I was never charged for her time with the 2nd application and called her because I didn’t hear from the review board after several months and she told me not to do a thing! It could be sitting on someone’s desk collecting dust and since he had medical assistance they can’t cut it off until the review board and all your appeals are exhausted. In Pennsylvania we have the “PA LAW PRODJECT” who is there to help parents of children with disabilities. And there are more! You can also contact your local Children’s Hospital for information on how to get medical assistance ask your doctor’s they have a wealth of information. Remember I am from Pennsylvania and your state is probably different. I was very fortunate to have all of the leg work done for me. I spent hours on paperwork and my sister and an attorney gave me all the information. It is a up hill battle but it is one that can be won. I hope this helps....DianeDiane
