awful virus

We have been housebound for a week!!! My 3 year old CAH son woke up in the middle of the night with a 102.8 fever and was vomiting. I was able to get him to keep down a stress dose of Cortef, so I thought we would treat it at home without having to give him a shot or take a trip to the ER. The next day, he had more trouble keeping the Cortef down, since he hadn't eaten anything for over 24 hours, I sense the Cortef was upsetting his stomach even more. I called the pediatrician who wanted me to bring him in, but I felt that we should just give him the injection at home. Our pediatrician said to keep trying the meds orally, a little at a time. So now my 2 year old daughter with CAH has the same thing, but she is looking really bad. She has been keeping the Cortef down and she is drinking lots of water and gatorade, but I wondered about a few things: 1. The signs and symptoms of adrenal crisis are so simiular to that of a flu or virus, i.e. loss of appetite, fever, lethargy, vomiting, that I am afraid that I won't know before it is too late. When do you give the shot? I thought we knew, but they were able to keep the Cortef down when I gave it to them a little at a time, which leads me to my next question... 2. Is the Cortef going to them as much good if I have to give it one ml every half hour, or does the stress dose need to be given all at once? I feel like if I had given them shots, they would have pulled through faster, but is that really important? They are pulling through it but it would help to hear from people who have been through it.
