Re: Prenatal dexamethasone treatment for CAH

Andrea, I too was given alot of information to read when my first CAH child was born, because we were planning on more children and we were urged to use the Dex treatment. According to all the literature, you are right, it is safe for you and the baby. Even if you are carrying a non affected boy, you shouldn't expect any problems. However, the biggest concern is the list of side effects to you. Of course, you may not experience any side effects and you won't know until you start the treatment. It is my understanding that any side effects or problems are reversible or treatable. I did not know I was pregnant with my second child until I was 8 weeks along, because I had just had my son, and I had not had a "normal" period yet. By the time I found out and got in to see the genetic counselor, I was 10 weeks along. I started the Dex at 10 weeks, but I was too far along for the CVS analysis, so I had to wait to do an amnio at 16 weeks. In fact, I was carrying a girl and she did have CAH, so I had to remain on the therapy throughout the entire pregnancy. I can tell you that I had the worst 9 months of my life. I gained 50 pounds, I was borderline anemic, VERY moody and tired all the time. I would go from being joyfull to depressed in a wink. It was an emotional roller coaster. I developed ugly red striations, like stretch marks all the way down to my ankles, around my hips all the way around to my butt, and all around my stomach. My face looked like someone smashed a frying pan against it and flattened it, I developed the moon face and I looked puffy and swollen all the time. I did not have any trouble with blood pressure or gestational diabetes, 2 common side effects of the Dex. But every woman is different. When my daughter was born, she had a moderate case of virilization, and she required 2 surgeries. It is hard to know if I helped her very much. I know I could have helped her more if I had started the Dex in the very beginning. When I became pregnant with my third child, I knew that I would not be taking the Dex. I couldn't afford to be emotional and exhausted with 2 toddlers running around the house. I was lucky, because my third child is a boy and he does not have CAH. I amstill trying to lose the weight-it is taking a while. The striations have faded, but they are still visible. I thought that one of my doctors mentioned that it could take a year or two for your body to return to it's normal state after being on the Dex, but you might want to ask about that. I hope I didn't scare you. I knew what the side effects could be before I started taking the Dex, and I was willing to give it a try if it would help my daughter. I will say this, my son had extremely elevated levels when he was born, and he needed to take sodium chloride for the first year of life. My daughter, the one I took the Dex with, had much more controlled levels right after birth, and she did not have to take the sodium chloride, even thought they are both salt wasters. Good Luck to you and please feel free to ask me anything. Nicole
