The toll free phone number for MAGIC is 1-800-3MAGIC3.They have an answering machine if you cannot get a real person. Our contact person and "ring leader" with MAGIC is Cathy Kusch. She is great. She will call you from her home if you need to talk or just want info. You can leave your name and address and they will send the ER sheet in the mail. I was the one who had to use it recently while we were at the yearly MAGIC convention. The doc went right by what it said because he really had no clue what he was dealing with.
