My endo gave me a letter that gave pretty specific instructions. I.e....if vomitting, give IV with exact meaurments of how much and what based on his size, etc. I am sure if you gave your doctor a call, they would gladly make another copy for you. Some generic form would probably be taken less seriously than something that leaves little guesswork. That is not to say an information sheet on CAH would not be of benefit...but I feel very comfortable with the letter I carry...they followed it to the letter when Kyle had rotavirus.
