I'm not sure I can answer all your questions, but I'll answer some and maybe someone else will do the same. I realise it must be tough for you to be dealing with all this. CAH IS genetic, it doesn't just develop the child must have inherited two mutated genes from the parents. Some cases of CAH are worse than others, it depends on how overgrown the adrenals are so some children produce more cortisone than others. The bone age will increase suddenly if the andrenals are not well in control, so it is not surprising that your sons bone age increased. I think this is often how late-onset CAH is picked up (by increased growth). There are loads of sites that you can get information from on the left of this board.
