I don't really know what advice to give you except that maybe you should contact the Magic Foundation to see what kind of recommendations they can make. I don't understand the formula you are using to calculate the cortef. When my daughter has a low fever and isn't feeling well, I give her twice as much medicine as she normally takes. If she is very ill with a fever over 101, then I give her triple doses. She takes 5mg of cortef in the morning, 2.5 mg in the afternoon, and 5mg at bedtime. She also takes florinef (.1 mg in the morning) for the salt-wasting problem. I am very sorry to hear about your son. He may have had CAH also, which can be fatal if untreated and is sometimes not detected in newborn boys because there may not be obvious outward signs of a problem.
