The packet didn't have the emergency instruction sheet included but there was a blurb in the newsletter under the CAH section, about getting one by mail by calling 8003MAGIC3. I have printed out the info on this board that was posted awhile ago regarding emergency instructions. Haven't had endo make up one as of yet. Going for a consult with a new endo soon!
