Our pharmacist told us that we only need to give it with food if it seems to bother their stomach, and our doctor and endo agreed. We crush it and give it with 1cc of water in an oral syringe - we've done this with both of our CAH kids for almost 5 yrs and have never had any trouble (and they don't wake up most of the time either!).
