Might be a matter of thinking whether the IM is needed (for immediate action into the system) or that extra oral is given (considering the oral won't start working immediately or that with the fainting or whatever else is going on the absorption system from the stomach may not be working well enough). Might the pain have caused that to be an adrenal crisis, maybe low blood pressure?
