I am glad that you have an appt with another endo. I really think that is the key. Once you have a good dr. they can work with you to find the right meds and doses that work best with your body. I feel for you about the exam part. I have not been through what you have, but I did have some traumatic experiences and I know how they can scar a person. What do you think about not mentioning the "don't want an exam" until your appt. instead of at the time of scheduling. That way you would be in the dr.'s office. Sometimes I think a face to face explanation (or statement) promotes more understanding that a phone message relayed from the person scheduling the appt. Face to face, I would think the dr. would be more understanding of your situation and less likely to think you have an "attitude". When I was "shopping" for a new endo for my son, I didn't say much at all on the phone. I waited with all my questions until I was in her office. I liked her and she accepted my son as a patient. (even though she wrote in his chart that I am a little compulsive! hah - she doesn't know me well. I am very compulsive!) Good luck and I hope you continue to visit the board. I think everyone here will do what they can to help you.
