Florinef - To Theresa

Theresa, As Cindy has pointed, out florinef is not salt, it is a synthetic hormone that causes the body to retain salt by mimicking the function of aldosterone. In the most common form of CAH both cortisol and aldosterone synthesis are blocked and 17-OH progesterone (17-OH) levels become elevated. It may be that in some non salt wasting forms of CAH, aldosterone synthesis is inefficient but enough is produced to prevent salt wasting. By inefficient I mean that some of the 17-OH progesterone being produced in the attempt to make aldosterone is not being converted into aldosterone. If the amount of 17-OH progesterone left over from this inefficient aldosterone synthesis is high enough to significantly impact overall 17-OH levels then it makes sense to administer either supplemental sodium or florinef in order to suppress this contribution to those elevated levels. Why one doctor prescribes florinef and another doesn't may depend on how efficiently the child is synthesizing aldosterone. I hope this helps. Joan
