Dear Marguerite, You asked what you might expect with your daughter's upcoming tests. This was our experience: With the ACTH test, the procedure was the following: With the first needle stick, enough blood is drawn out to get a baseline reading for a number of different hormones (our endo tested for about 7 adrenal hormones, as well as ACTH, potassium, sodium, renin, and a bunch of other stuff.) While the needle is still in the arm, a synthetic hormone is injected into the bloodstream to stimulate the adrenal glands to make cortisol. Then the needle is taken out and a second blood drawing is taken one hour later. The difference in values between the second blood drawing and the first, enables the doctors to make a determination of CAH. With the LHRH test, the idea is similar, but the procedure is slightly different. When my son has this test done, he has to sit with a needle in his arm CONTINUOUSLY for one hour. (The lab had videos for him to watch, and I made sure to have a lot of snacks on hand, so he could munch his way through the one hour period, as a distraction.) Again, blood is drawn and stimulant hormones are injected through an IV. Measurements of two hormones called LH (Luteinizing Hormone) and FSH (Follicle Stimulating Hormone) are taken at 0, 15, 30, 45, and 60 minutes. The values returned at each of these times helps the doctors make a diagnosis. As others have mentioned, the ACTH stim. test is for diagnosing CAH, while the LHRH test diagnoses puberty. Puberty begins after the adrenal glands have matured, or reached something called adrenarche. In a CAH child---especially someone who is diagnosed on the late side---early puberty is sometimes a problem because the adrenals have been allowed to mature prematurely. We were able to get some of our tests back rather quickly, and had to wait for others---I think it would depend on the lab that you are having the work done, but you will not get anything back that day. In our case, we were lucky to get the key numbers back within a few days, so that we knew pretty much right away, it WAS CAH. But it took a couple of weeks to get every single number back and flesh out all details. I hope this helps you understand, at least, the test procedures. The best of luck to you and your daughter.
