It's actually a gold St Jude medal about 2in x 1in I found at a shrine in New Mexico while out on an assignment.  I took it to a jeweler when I returned home to Atlanta and instructed him what to engrave.   I wear it on a plain gold chain.  I would send a picture if I had one but alas, do not. 
 
I think that anything they found fashionable that can be engraved would work just fine.  I know when I go to music festivals and such, there are always tables set up with jewelry vendors selling the kinds of things kids like to wear.  I guess my point was that it doesn't need to be an "official" tacky med alert item but can be any type of engraveable jewelry that is large enough to engrave with the information that you felt necessary to include.  A doctor I had some years ago suggested it when I griped about how tacky they were.  Granted, there is some trust in it being noticed but having asked medical types along my journey about it, learned they usually check any jewelry for information and identification.  I hope this helps.
