I like the John Hopkins CAH pamphlet (free, on the internet or you can call them), the Texas Dept. of Health brochure, and the PENS CAH & adrenal insufficiency brochures.  The MAGIC Foundation brochure does not mention the term adrenal crisis, nor state specific emergency instructions in it ... so if you use theirs, also get a copy of their emergency instruction sheet (you'll have to ask for that, they don't voluntarily send it out!).
