You know, my daughter complains of this sometimes, too, and when I take a look, the urethral opening looks quite reddened. We have had great success with a zinc barrier cream (such as Zincofax) applied well around the opening - it takes the "ouch" away almost immediately, and if she complains at or around bedtime, I put a good bit on her at night before she goes to sleep; vaseline would also provide a good barrier between boughts.
