Gary, you used the phrase "a feeling that I know my daughter well".  It has taken me much too long to realize that I know my children better than any doctor, and I am the one ultimately responsible for optimizing their care.  I strongly believe that our opinions and observations are much more important than general guidelines that a doc is going to be following.  There is so little consistency in standards of care anyway!  I would encourage all parents of CAH children to keep a firm backbone when dealing with your medical caregivers.  We are pleased with our current ped endo, but it took my very strongly stated concern to make her really address the weight problem. 

When my children were first diagnosed, we were with a ped endo who was not competent to handle CAH.  One of many mistakes she made was miscalculating my girls' body surface areas.  The result was that they were grossly undertreated and we were at a loss to understand why they were not responding to the meds as expected.  I researched and discovered her error, but she refused to even entertain the idea that she had made a mistake.  Naturally we found another doctor.  Question everything!
