Since my daughter (CAH SW) is only six, I don't know how well I can answer some of your questions, but yes, the dosages will change with growth, etc. That is why it is imperative to work with an endo who believes in regular (in our case, quarterly, more often if problems) lab work, height and weight checks (growth chart), etc.

Because of the salt-wasting aspect of the condition, we really push fluids (water, gatorade) when she is active, such as playing soccer, when it is especially hot,  etc. We really work at keeping her hydrated and are encouraging her always to pay attention to how she's feeling. However, we would never keep her from participating in anything just because of this condition.

And no, small scrapes, cuts, bruises do not require extra cortef. It's the big stuff, such as fevers, illness, broken bones that require extra meds and/or an injection of solu-cortef.
