Well Cindy, my sister-in-law was just diagnoses with a thyroid problem and she exhibited many of those same symptoms. A deep voice, increased bowel movements, bad headaches, dark circles under her eyes and lethargy. She also had some muscle wasting in her legs and she got very skinny. She had discomfort in her abdomen so bad that she thought that she might have cancer. The headaches were so bad that the doctors had begun to treat her for a sinus infection. Is his thyroid enlarged? These symptoms sound all too similar to me, but I am sure that the doctors have probably tested for this as it is a relatively simple test. I couldn't tell from your post if the child has CAH or you were just suspecting that he might, but if he does there have been many parents who have reported that their children with CAH have a sensitivity to wheat and starches. Were those pancakes made with wheat by any chance? Please keep us posted as to the outcome of your friends diagnosis.
