Our salt-losing CAH daughter got chickenpox at 18 months and had no trouble at all, just horrible nuisance spots.  We kept a close check on her temperature but no worries.  I wouldn't increase cortef unless really necessary, best to check with your endo.  It's too late to get the vaccination for your daughter now, but I think I will for our other daughter because they really are just a nuisance.
