The brilliant endo seemed to have a speech memorized about CAH people and illnesses (he was 65 years old).  He knew how quickly adrenal crisis and death can occur in adrenal insufficient people.  He said one reason to give extra cortef early on in illnesses is to prevent an adrenal crisis from ocurring.  About every other sentence of his speech used the term adrenal crisis.  His said you want to mimic the body's natural production of cortisol (as produced in normal adrenal glands), and which normal adrenal glands will produce extra cortisol with colds, minor fevers - so go ahead and double the dose with anything minor.  He said with even a cold, double the dose for the first three days of the cold and that it was very important to double for those first three days, to deal with the cold, help fight any possible infection that might occur (sinus or pneumonia) .  He agreed with giving extra for emotional stress, because in a normal person their adrenal glands will produce more cortisol to deal with that emotional stress.
