Our son in  9 years old and we have never given extra cortef for the normal stressors that you mentioned. After hearing what other parents on this board have done, I don't honestly know what is the right answer and really question who does know the correct answers. A lot of times it seems like everyones doctors feel a little differently than the next. It is a relatively unresearched disease and in many ways I consider all of our children to be the guinea pigs for the next generation. You will find loads of wonderful information and support on this board. Seeing that your son is older (no longer an infant) I would love an opportunity to ask you lots of questions.
