The fore milk is more watery and is faster because it is just the thirst quencher.  The hind milk is the real meal.  They need to be left on the first breast as long as possibly until they have drain the milk ducts.  Otherwise they do not get all the goodness.  I feel it has a lot to do with how your midwife is in hospital sometimes.  I have had a few that say 5 - 10 mins this side and then the same on the other.  But when you think about that, what you could be giving your baby is all the fore milk off both breasts and none of the richer hind milk.  My first baby was I guess my learning curve.  The other two (even thought I had a caesarean) where easier because I had been in touch with La Leche in the past and they are very good with these things.  I think things are slwly changing in Maternity hospitals because this last time I fed, the midwife was excellent.  Right from the start she knew how to help me with latching the baby on correctly and leaving him there to completely drain the breast.  Your totally reliant on that after a caesar and she was excellent.
