The reason why small children need to be on a larger amount when babies I have been told is because before they are diagnosed (can be up to two weeks) their adrenal glands enlarge so much and become hyperplasic and their blood levels fall very low where the sodium and other electoryltes are concerned. This cannot be recitied straight away, it can take a long time for their adrenals to regress and their blood levels to come back withing the normal ranges. Ashley was on a drip for two weeks, and I do not know what they dose him but after release from hospital he had 100mcg from four weeks old for definate (in suspension form) and it has stayed that way. the only renin tests that we had come back unusual was last month. It read that renin was high at 18.8* nmol/L. I do not know the normal ranges but the doctor rang me and said to switch him back to halving the tablets 12 hourly apart. However, I knew that I had inadvertantly missed three to four days of doing him the florinef, so I knew why his renin had been high on that occasion. Because we had gone to halving and quartering tablets and I was using a 7 day pill dosing box, I had been so busy the first week juggling the hct quarters and halves as they should be distributed in each protion appropriate for that partuclar time of the day, that I put the container away after that "without" putting the florinef in. I left him to adminster his meds himself (otherwise i would havepicked it up on the first day that he had no pink ones in the container!) and he did not bother that they were not there. By the fourth day however, he complained of nausea and I checked the container to make sure all of them during the day added to 15mg total daily doses. It was then that I found the error and realised. So when the doctor rang up, I knew that why his rening levels had been too high. I presume that they will look better next time. I know that high blood pressure is generally associated with too much meds, but if the child is swcah, then you really need to maintain the doses of florinef that have always been. The average person needs 100mcg at least. All I can say is, perhaps if the child has really high blood pressure like you say, he needs to be in hospital for a day or two and observed to try to get to the bottom of the problem. It's better to err on the side of safety to be honest.Val
