There are a few endocrine disorders (late-onset congenital adrenal hyperplasia and Addison's disease)  as well as poly cystic ovary syndrome that can cause high androgen levels and weight gain. You need to get her to an excellent endocrinologist. Where do you live? Please email me with your state and town and I will look through my references for a qualified physician for you.
