It would concern me if my child was on Florinef and he or she was diagnosed SV. Florinef taken at too high a dose would not only cause problems with growth, it may cause problems with high blood pressure also, if too much salt where being retained. It is a long acting mineralcorticoid also so in addition to being taken just to treat SV instead of using cortisone may cause growth as you mention. Most kids diagnosed SW are only taking it because they have the inability to make aldosterone anyway. I read the post below about some Endos believing that all CAH individuals suffer from some degree of Salt wasting aspect, but Hydrocrotisone in itself has minercorticoid properties that if given adequately should really address the sodium requirements of a simple virilizer. Surely if Florinef is being prescribed knowing that it is going to retain a certain amount of sodium for a child that can already retain reasonable levels, it will cause problems I outline? HCT retains sodium so much so that Ash only did six weeks off it lately and his renin levels were elevated to 18.8nmol/---(can you convert this Carol to ng/dl) which is quite high compared to the normal levels. As a child grows their demands for sodium increases but the Florinef doses remain the same---it is the hct dose increases that are relied upon to meet the individual childs needs---rather than raising the Florinef dose. When the child does sport or any other activity on a regular basis, they expend whatever electrolytes that they have retained and when the levels fall low in the blood that is when the craving occurs for sodium or glucose. Unless there is a sufficient amount of aldosterone present, all the sodium is secreted in the urine. Alternatively a SV on too high a dose of Florinef would be a problem as too much sodium would be retained, which leads to water retention and high blood pressure. I think I posted a while back and said that the cells actually burst as they retain too much fluid. Not good. Maybe the "cells bursting" bit is what causes the retardation of growth??? Cells need to regenerate for growth to take place do they not?
