As far as the pharmacy's role, I've experienced it a couple of different ways.  One pharmacy gave me the whole bottle, direct from the manufacturer, and just slapped their own label on top of it (even though the total amount was more than what the doctor had prescribed.) Other pharmacies are not quite as generous, and dole out the quantities more judiciously.  Either way, the pharmacy does not have a hand in the preparation of the dex liquid.  It comes from the drug company in larger quantities and the pharmacy transfers it to a smaller bottle, before we get it, but that's it.
