My now 4 year-old daughter also had extreme growth problems from the time she was born to the time she was 16 months old. The turn around for us was visting a Peds. specialist at the Mayo Clinic in Minnesota. We were not getting anywhere with our regular Endo. either. He seemed very unconcerned with her situation. At Mayo we learned that she simply needed salt. I would give her 2 tsp daily, divided in her cereal, etc. He also took her down from one 1ml dose of florinef daily to .05ml dose of florinef daily.
