We have ours in a HUGE binder sectioned off with colored tabs.

One for meds information (dosage, changes, shortage, anything) labeled "Rx", one for hospitalizations and treatment labled "PCH" (the initials for Phoenix Children's Hospital), one for growth and immunizations and regular pediatrician visits labeled "general", and one labled "research" which is anything clinical we've found, and "support" is the last tab, which has help from CARES, from Carol and Laura on Circadian Rythym info (which I am creating a new tab for "circadian ryth") under the Rx category.
