RobertaTake sure that they have him on monitors for blood pressure and heart monitors (demand that they use the monitors - if they say that they don't have monitors - find another hospital that does). I would think he will need solu-cortef and IVs before, during and after the proedure because of the sediatives. And stay longer in the hospital to be monitored (and gradually decrease the cortef back to normal - you can't get a big dose and then go back to a small dose - it is suppose to be gradually decreased back to normal).
My three year old had a chipped tooth at 15 months old and of course will not hold still for dental work. It is not bothering him, so we decided not to get anything else done with it. Some dentist said wait until he is 3 years old. I took him back at age 3 and again they didn't want to do anything with it because of the CAH.
My eight year old had small cavities and had very small fillings (back teeth). And no extra cortef (injection or IV) was needed because they just numbed the area... only if they are using drugs to put them to sleep. I gave double cortef (just in case) anyway. He did fine with it.
