My son, who is CAH non SW, is quite a bit older (16 years old) and had to have 4 wisdom teeth and 2 regular teeth removed to accommodate brace placement.  As he was older and a non salt waster, it was not necessary to place him in the hospital.  However, I made sure that the surgeon spoke with my son's endo directly so here would be no mistakes with dosages during the procedure.  As Roberta said, there were doses required before, during and after that were different from the usual dose.  After the dentist and endo spoke, I spoke with them individually to make sure we were all on the same page.  The dentist was excellent, had heard of CAH (not always the case) and contacted me again before the procedure (which I would have done if he hadn't) to go over everything again.  In my son's case, the medication they used to prevent excess swelling was dexamethasone (a treatment for CAH).  Therefore, since he was going to be receiving high doses of dex (stronger than cortef) the dose changes were made accordingly.  That being said, my advice is to question everyone involved and to make sure the dentist and the endo speak directly.  Good luck!
