My non CAH 8 year old daughter used to do that at age 5 and we found out she had a massive wax plug pressed against her right eardrum.  It was disgusting and was shaped like a scallop.  This was due to repeated ear infections, particularly in that ear.  It was painful for her and kept waking her up, and she would act out when at home and there was too much noise (tv while people talking etc).  She had no idea what was wrong and neither did we.  It took a really smart pediatrician to figure it out through a bad result on a pre kindergarten hearing screening.  Otherwise we never would have known.  By the way... her hearing returned to normal a few months after it was removed.  It's worth a look.  Just a thought.
