I have SW-CAH. I have 3 children (guaranteed carriers) and number 4 is due in 5 weeks. My husband has NOT had genetic testing. He did have an ACTH test where blood was drawn, cortisone was injected and then blood was drawn again at 1-hour, 2 hours, 3 hours. These results indicated that he is not a carrier. I did not opt for CVS and/or amnio. Don't really know why. We figured we would cross that bridge when we got to it.
As mentioned, your wife needs to be in good control in order to get pregnanct. In all my pregnancies, my steroids have gone up 3-4 times the normal dose. I started out at 0.25mg of dexamethasone, 0.1mg of Florinef and 0.088mg of Synthroid. I am currently at 0.75mg of dexamethasone, 0.25mg of Florinef and 0.125mg of Synthroid. Your wife should expect to have blood drawn montly and to visit her endo every 6 to 8 weeks in the beginning to 4 - 6 weeks at the end. The OB can order the blood work. She should report any nausea ACCOMPANIED by muscle weakness to her endo immediately. This is a sign of undersuppression. If your wife is undersuppressed for a large portion of the pregnancy, the baby maybe underweight (this happened with number 3), not to mention the possiblility of adrenal crisis.
I did have all by c-section, some of which I think may be because of CAH. I've never experienced labor.
I did breastfeed all of them and I intend to breastfeed this one.
