Dear All,

My 5 1/2 yr old daughter (non-salt wasting CAH) has been wearing a medical alert necklace and bracelet.  (We bought both because she had already lost the necklace.)

On the necklace, the only choice was "Adrenal Insufficiency."  However, on the bracelet it allows us to print whatever we wanted.  Any suggestions?

Somewhere, I noticed that someone said what to do in emergency, but now I cannot find that post. 

Also, what else should I have on a medical release form that I plan to have notarized with all pertinent info such as phone numbers, doctors, who is allowed to seek medical information, etc.?
