I work in the medical billing field.  It is hard to give you a lot of advice because I don’t know the whole story, but I have a few pointers for you.  For one, you may want to get a letter of medical necessity from the doctor that provided this service for you.  I am not sure of course what their reason is for paying such a small amount, but you can always appeal their decision, and I believe the letter of medical necessity would help out with that.  I would like to say in many cases the billing office codes procedures correctly, but you may want to verify that with them because mistakes do happen.  Depending on how claim was processed by BCBS, you may also want to ask the billing company if they have filed an appeal because if BCBS paid below their contracted rate, it is the responsibility of the billing office to chase down that money.  I am not sure if that helps, but  hope it will push you in the right direction.
