Actually it would be 5mg each dose over three times per day.  I think you said that the dosing was 10mg pm 5mg morn?  I think really when Doctor is away and not able to advise and labs have been so delayed coming back, I would not be taking ANY chances with my childs health.  I would not wait for a Doctors permission to double either.  Ashley has been ill on many occasion and I have asked and waited and the outcome as been worse on every instance.  He has gone into crisis.  it simply is not worth the hassle as it can take higher doses to pull them out of one.  His med’s would be doubled and I would not really worry about the next lot of labs being out as long as he recovered well.  In the last illness we found the larger doses did enable him to play and he looked fine.  However, as the day wore on he would be become quite ill.  

In my opinion it is of little use to spread the doses out unless you double up because by distributing the total daily dose there are areas in the day where the doses are less than what they should be in illness.
