This pamphlet uses the term testosterone to refer to all androgens.  It does not differentiate between the androgens produced by the adrenals and those produced by the testes.  So, in the terms of this pamphlet, testosterone is elevated in undertreatment and this causes infertility, but it’s the testosterone produced by the adrenals!
