Why not get another’s doctor’s opionion first?  My kids are 4 and 8 years old and have salt wasting CAH.  They are doing VERY well on the cortef three times a day and florinef in the am.  I do not think that they would have on cortef twice a day.  Cortef is very short lasting and only lasts in the body 6 to 8 hours so do the math.  There are 24 hours in a day and not 16 hours. 

Maybe - for the non-classical and mild CAH kid - twice a day is fine but not for the classical CAH kid. 

I would also consider going the National Institute of Health and checking to see if they would see your daughter.
