Rich,  I am an adult with CAH and am also self-employed. I have Blue Cross and Blue Shield (in NJ) and there haven’t been any coverage problems, other than the monthly bill which is about $400.  Make sure you check any clauses that concern lapses in coverage.  When I came off an employer sponsored plan, I had 63 days to get my own (which is US COBRA law) in order to avoid having pre-existing conditions included.  I’m not sure how it would be converting from a foreign plan.
