I have twins with SWCAH, they are 11.

I do the same as Martha, I try to stay away from the hospital as much as possible. When throwing up I ALWAYS give the shot. My Endo says that even if they are holding down the pills that the stomach can not release the meds and they can go into an adrenal crisis. After the 2nd shot the Dr. usually wants blood work to make sure salt levels are OK. That has happened only once. I give Gatorade in small sips ,they hate pedalite. So I figure gatorade is better then water, and that one time he took blood he was amazed that the salt was in the OK level.

I triple the dose when fevers, till 24hours after the fever goes down, then double for another 24hrs, then back to normal doses. I also give the triple dose every 6 hrs. For instance normal doses 25mgs.  Triple 80mgs over 24hrs giving 20mgs every 6 hrs.
