I know there are many variations -the one we’re using I got from the mom/coordinator of the CAH group through Magic.  It says "Adrenal Insufficiency, Give Hydrocortisone Injection in Emergency" -our endo said that sounded "right."  Hopefully others will post again as I know there are lots of others out there.
