robertaAny fever 101 - immediately double or triple the cortef. My kids didn’t get fevers with teething. But if they did - I would have tripled the cortef and given motrin or tylenol because they are severe salt wasting CAH kids. And I would have tripled the amounts for 48 hours and then tapered off over the couple of day. The next day doubled... then normal. I would have only given the solu-cortef injection if there were other problems - fever would not reduce after giving motrin or fever reducer after 30 to 45 minutes. Tylenol lasts about 4 to 6 hours; Motrin - 6 to 8 hours. Write down the times you gave meds and make sure you repeat it. Repeat the triple/double dosage of cortef every 8 hours if mild 101 fever. If higher fever and more severe infection - every 6 hours. Any other symptoms like - too sleepy, hard to arose; listless (like a rag doll); dizzy, won’t eat or drink; etc - then I would get solu-cortef into him immediatley and give sodium/sugar IVs. Sugar and salts could be down.
If the fever goes higher than 101.5 - YES - give the solu-cortef and then go to the hospital for IVs. Especially if the child has acting listless. The blood pressure could be low; and he could be shocky - and die. Sodium and cortisol imbalance. You can’t over dose from the cortef but not enough - shock and death. GO TO THE HOSPITAL and explain that he could be in adrenal shock and could die!!
