Chris DMy 5 year old son’s 17OHP levels often come back <10ng/dl (or undectable). He actually hasn’t had a reading over <10 in a year. Because he is on a relatively low dose and his growth is good, his endo does not want to reduce his dose of Cortef. When we have reduced his Cortef in the past, he has growth spurts and shoots up too quickly.
We also have his blood drawn after morning meds, as close to 2 hours as possible. From what I can gather, as long as you are consistent in the timing of the draws (and it sounds like you are), the endo is comparing "apples to apples" and should be able to make adjustments in expectations for where the levels should be. Given the draw is after the morning meds, I would think it is to be expected that her 17OHP is low.
I think it is a good sign that your daughter’s dr. is taking into account more than just the blood levels. Since your daughter’s growth is good and she isn’t showing physical signs of oversuppression, it sounds like you just might be very fortunate to have found the right dose for her.
How many mg/m2/day is she on? The consensus statement, as well as other articles I have read, say the typical dose is 10-15mg/m2/day. As long as she is not on significantly more than that dose, I would think everything is going well.
I’ll be interested to hear what others have to say though. Our situation is similiar (good growth and low 17OHP), but my son does show some signs of oversuppression - weight gain.
