When it comes to dosing, you really need to "nudge" them. If they object to being bothered, then you should find another doctor. By law (US law) you are supposed to administer dosing exactly as the doctor instructs. If a docter were to in any way discourage legitimate questions regarding dosing he’d be opening himself up to a mal-practise suit.
