Have you or the ped endo considered having the Cortef/hydrocortisone compounded at your local phamacy into a suspened liquid.  That is what we do, my daughter is 4  years old and has been on the compounded liquid hydrocortionse for almost 3 years and has been doing great on it.  My daughter also takes a small amount of hydrocortisone so her ped endo prefers her to take it in a liquid form so we can make small adjustments when a dose change is needed.  If you have any questions about it I would be more than happy to answer them for you.  You can e-mail me or post here.  I hope this helps.
