My 2 month old son is on 2mg of cortef 3x daily.  We have a pharmacy compound the 5mg tablets into 2 mg capsules.  We simply break open the capsule (which is crushed cortef) add a touch of water and give it to my son.  As for the cost, there is no extra charge.
