high cortisol and low aldosterone?

According to figures 2 and 3 at http://aafp.org/afp/990301ap/1190.html
21-hydroxylase and 11-hydroxylase deficiencies result in low
levels of *both* cortisol and aldosterone.

But when I take cortef to supplement my cortisol levels, my headaches
and dizziness are worse. When I take florinef to supplement my
aldosterone levels, my headaches and dizziness lessen.

In AH, can aldosterone be low but cortisol normal/high? Would that be
an 18-hydroxylase deficiency? Is there such a thing?
