Our 5 year old son also can be quite moody. He has gone through periods where it was so bad I was sure it had to be related to CAH and its treatment - he was just way too emotional for it to be normal toddler/little kid behavior.

Last May we switched his dosing schedule. He now gets Cortef 4x a day. We have noticed a dramatic difference in his moodiness. He is still emotional, but he is in control and can pull himself back. It is very different from his extreme distress of the past. I am sure some of this is because he is getting older - he turned 5 this summer. But I can’t believe it is just a coincidence that we saw dramatic improvement following the dosing switch.

Have you noticed any patterns in your son. Does he get more clingy and distraught shortly before the next dose? When he is hungry? Our son, Jack, was at his worst around 5pm (when he was hungry and due for his evening dose). I now believe Jack metabolizes the Cortef more quickly then some others. He really does better on smaller, more frequent doses.  Just a possibility. I know others on the board find a 3x, or even a 2x, a day schedule works fine for them.

Good luck. (Sorry I can’t help re: the acne)
