Congratulations Michelle!!!  I also have 21-hydroxylase, SW-CAH.  I’m 39 and have 4 children (ages 10,9,3,1).  I normally take 0.25mg of dex and 0.1mg of Florinef.  In each of my pregnancies,  my dex has slowly crept up to 1.0mg (0.25mg am and 0.75mg pm) and my Florinef would go up to 0.2mg during the course of the pregnancy.  I’d end up to 1.0mg by the last week of the pregnancy and would stay there through the delivery (c-section) until about 5-7 days after.  I would then slowly go back down to 0.25mg dex and 0.1mg florinef (3 days of 0.75, 3days of 0.5, 3 days of 0.375, then 0.25).  During delivery, I would also have hydrocortisone via IV on delivery day.

I don’t know if that helps you since I wasn’t on Cortef and I don’t know what the doses translate to (how much cortef = 1.0mg of dex, etc.).
